Supporting Young People To Thrive

Evolve Care & Support provides semi-independent housing with floating support for vulnerable young people between the ages of 16 to 18 years who would otherwise be homeless or exposed to substantial risk without our services.

We provide semi-independent accommodation and floating support for this category of children and young people within various local authorities where they are domiciled. 

Our services Include:

  • Providing suitable, safe and secured semi-independent accommodation

  • Conducting need and risk assessment

  • Support in finding approved foster carers in the absence of suitable family member or guardian

  • Support them with emotional, language and adaption to local environment UK

  • Signposting to services and external support

  • Support with skills acquisition, education and recruitment

Transition Support

We also provide supported accommodation options for young people who are ready to transition into more independent living arrangements. Through this program, we empower these young individuals with the necessary skills and resources to thrive on their own while still receiving ongoing support from our team. Every young person will work with their key worker to get the support they need to develop independence, self-reliance, and resilience for their future endeavours.
